Adduction gait

Walking with the feet inwards is a deformity that we frequently see in our podiatry clinic in Malaga, it is benign and consists of a deviation inwards of the front part of the foot.

 

Visually, the foot is bean-shaped (with a convex outer edge and a concave inner edge), and can cause some problems when putting on shoes, especially if the deformity is very accentuated, since it will present excessive pressure in that area, which can lead to areas of overload and wear and tear of the footwear.

 

Genetic, mechanical and postural factors are involved in this deformity, both intrauterine of the baby during pregnancy, as well as with the positions adopted by the child during sleep or play (sleeping on the stomach favors this deviation). Forefoot adductus is strictly related to adduction gait, and when the child begins to walk, it is seen as “walking with the feet inward”, which can cause loss of balance, instability and frequent falls.

Although the cause may not only be the forefoot, it may be caused by abnormal torsion at higher levels of the leg or hip, resulting in the same type of inward gait.

 

As for the evolution, it is flexible, sometimes it corrects spontaneously as soon as postural habits change, but if this is not the case, or the deformity is very evident, it will be necessary to place insoles, in addition to reinforcing with stretching exercises and manipulations of that area to rebalance the musculature involved.

 

The insole will be formed by a piece that will be placed in the external area of the foot, which will make the soft parts adapt to this new corrected position, thus modifying the mechanism and the angle of deviation and making the patient walk with straight feet. In addition, it will be essential to correct the postural habits that are accentuating this deformity, and to perform exercises that strengthen the affected musculature.
